If you're not sure if you or a friend has it or not, it's basically a disorder that causes people to want to be alone and away from social interactions (although it sounds similar than agoraphobia it's different) and generally have very odd ideas and actions. In a way, it is close to borderline personality disorder in the fact that it can make understanding reality pretty difficult.
So, what are the treatment options? Well, like other personality disorders Schizotypal sufferers can try to get rid of it with psychotherapy. It can be effective though it's very costly over time.
Obviously, you don't want to be spending the rest of your life in psychotherapy. It's just not any real way to live. Unfortunately, after trying many other options, this seems like a last resort. What really makes things worse is the lack of close friends that usually accompany one who suffers from this disorder. Though having a family to confide in is nice, most people need more than that. Having a very large support group is good, but considering that sufferers of schizotypal personality disorder engage in social isolation, they usually don't have that option.
To make matters worse, being paranoid is usually an effect as well. This is another difficult thing that can be made better by facing it with friends, though without them it can be very hard.
So clearly, it's a serious disorder, and one of the many of DSM-IV. However, you need to remember that you're not alone. It is possible to be completely cured of it and be free, so you can enjoy close friends that you can share your hopes, dreams, and fears with. You can enjoy reality and fiction when it's appropriate, and you can live your life like so many others take for granted. Don't lose hope, and keep your family members close.
